New Patients

How to register

Call in at the surgery and pick up a registration form (GMS1) from the reception desk. Fill in the form and sign it at the bottom. Get it back to the surgery and it will be processed immediately. Your previous practice will be notified and your medical records will be forwarded within about 6 weeks.

You can make an appointment at the surgery as soon as you hand back the registration form.

Unacceptable behaviour

Unfortunately there are circumstances when a registered patient is removed from the surgery's list of registered patients. Some of these are:

• Violent/aggressive behaviour, both verbal and physical.
• Damage to or theft from the practice premises.
• Smoking or the consumption of alcohol/illicit drugs on the premises or attending in an inebriated state.
• Inappropriate use of the emergency out of hour’s service.
• Any behaviour that significantly negatively affects the Doctor/patient relationship.
